Client Services Coordinator

We're looking for a Client Services Coordinator. The Client Services Coordinator is responsible for client intake, service coordination, and quality control assurance.

Qualifications:

  • Must have a high school diploma or GED
  • An associate's or bachelor's degree in human sciences is strongly recommended
  • Must be at least 18 years old
  • Must have at least one year of experience as a DSP, CNA, or HCA
  • Must be proficient in computer use and programs such as Word and Excel
  • Must be able to follow instructions to complete work as assigned
  • Maintains a current state driver's license and a dependable insured automobile
  • Demonstrated self-motivation, self-direction, organizational skills, flexibility, and the ability to coordinate multiple tasks simultaneously
  • Presents a neat appearance and positive work attitude

Responsibilities:

  • Client Intake:
    • Answers inquiries from potential clients about our services
    • Follows up on referrals after initial contact, weekly follow-ups for up to 6 weeks
    • Schedules in-home visits and meet with potential clients to go over services that Vibrant offers
    • Perform intake assessments
    • Develop client care plans and support plans
    • Discuss with the staffing coordinator about the staffing needs of the client (hours of service and caregiver skill level) for the scheduler to match the client with the appropriate caregiver
    • What are the skills needed for the caregiver to work with this specific client?
    • On the first day of service, meet the client with the caregiver, introduce them to each other, and go over the support plan
  • Service Coordination:
    • Acts as the primary contact regarding client services
    • Answer questions from clients and caregivers about ongoing services
    • Review service notes and data daily and follow up on any concerns you find in the notes (MAR, ISP data, Sleep Logs, T-logs, etc.)
    • Discuss with the staffing coordinator the staffing needs of the client for the scheduler to match the client with the appropriate caregiver
    • What skills and availability are needed for the caregiver to work with each specific client?
    • Assist as needed in the hiring and orientation of new employees. Participating in interviews will help you decide if the caregiver is the right fit for the client
  • Quality Assurance:
    • Schedule and perform client supervisory visits
    • Schedule and perform aide supervisory visits
    • Participate in ongoing orientation and training of new staff with clients
    • Takes a turn to be on-call for emergencies
    • Cover shifts if the agency cannot find coverage and the client cannot be left alone
    • Monthly: Review and write a client quality assurance note (ISP goals, sleep logs, MAR, ISP, Activities, etc.)
    • Quarterly: Schedule a care conference meeting between Vibrant, the client, and the guardian to review how the service is going. Address client goals and progress, challenges, medications, activities, current needs, etc. Write a quarterly quality assurance note
