Working under the guidelines of Hamilton Health Center's patient-centered model of care, the Patient Benefit Services Coordinator serves as a liaison for uninsured and underinsured patients, providing eligibility screening, enrollment assistance, education, and outreach related to Medicaid, CHIP, and Marketplace insurance programs. This role supports access to care by assisting patients with insurance applications, financial assistance options, and referrals while participating in in-reach and outreach activities to improve engagement in evidence-based care.

The Patient Benefit Services Coordinator is responsible for a wide range of duties, including but not limited to the following:
High school diploma or GED required. Coursework or degree from an accredited college or university in Social Work, Public Health, Public Administration, or a related field preferred.
Minimum of two (2) years of experience working with children and families in public health, outreach, social work, or a related field. Working knowledge of Pennsylvania Medicaid, CHIP, and the COMPASS benefit application system preferred.
Reliable transportation required to travel throughout the service area.
