Clinical Liaison/Account Executive

The Clinical Liaison/Account Executive (LPN/RN) is responsible for driving business growth by developing and maintaining relationships with referral sources, including assisted living facilities (ALFs), skilled nursing facilities (SNFs), hospitals, physician groups, and other community partners throughout Southern New Hampshire. This role combines clinical expertise with business development to assess potential clients' needs and connect them with appropriate non-skilled home care services. The ideal candidate will have strong sales acumen, excellent communication skills, and the ability to represent the company professionally in the healthcare community.

Essential Duties and Responsibilities:

Business Development & Referral Management:

  • Identify, cultivate, and maintain relationships with key referral sources (ALFs, SNFs, hospitals, physicians, case managers, social workers, etc.).
  • Conduct regular visits, presentations, and educational sessions to referral partners to promote company services.
  • Attend networking events, health fairs, and community outreach programs to increase brand awareness.
  • Track and report on referral trends, market opportunities, and competitive activity.

Client & Family Engagement:

  • Meet with potential clients and their families to assess care needs and determine eligibility for non-skilled home care services.
  • Conduct in-home or facility-based assessments to evaluate client conditions and develop appropriate care plans in collaboration with the care team.
  • Educate families on available services, payment options, and the benefits of private-pay home care.
  • Facilitate smooth transitions from facilities to home care by coordinating with internal teams.

Clinical & Compliance Oversight:

  • Ensure all client assessments and care plans align with company policies and state regulations.
  • Provide clinical guidance to non-medical caregivers when necessary (within scope of practice).
  • Maintain accurate documentation of client interactions and referrals in CRM systems.

Qualifications and Skills:

  • Licensure: Active LPN or RN license in [state].
  • Experience: Minimum 2+ years in non-skilled home care, home health, hospice, case management, or healthcare sales/business development.
  • Skills:
    • Strong sales, negotiation, and relationship-building abilities.
    • Excellent clinical assessment and communication skills.
    • Ability to work independently and meet business growth targets.
    • Proficiency in CRM software and Microsoft Office.

Physical Demands:

  • Must be able to spend long periods of time driving or sitting.
  • Requires occasional standing, sitting, walking; using hands to finger, handle, or feel objects, tools or controls; reaching with hands and arms; climbing stairs; hearing; talking.
